Our Vitality Program

The Vitality Passport, a program designed to take you on a journey to a healthier you. The Vitality Passport will provide you with the tools you need to make a big difference in your quality of Life. This is a proactive program designed to improve your resilience and assist you to become more robust, so you can continue to the things you love for longer. The Vitality Passport is a new, evidenced based program that aims to:

  • Improve Strength & Balance
  • Provide advice and strategies to manage Mental Health
  • Improve Nutrition
  • Improve Cognition
  • Assist you to be more active within the community.

We anticipate you will have the confidence to do more, and be more. Our hope is that as a result of the program you will live a longer and happier life, participate in more community groups, community events, and enjoy living. We expect to give you the tools to stay at home, and continue to do the things you want to do.

What Is Involved?

Individual One on One Consultations

You will have the opportunity to have three one on one individual appointments to assist you with developing a specific plan for exercise, nutrition and cognitive stimulating activity. That is you will receive an initial appointment, review appointment and a discharge planning appointment with each clinician. The consultations are usually 4-6 weeks apart. In order to make it easy for the participant to see each clinician we off the appointments at the same time, in the same place.

Building Physical Reserve - Physiotherapy/Exercise Physiology

Our clinicians will help develop and design an exercise program to assist you to become more robust, or build physical reserves. Often the goal of these sessions is to either introduce you to exercise or progress exercise prescribed at an appropriate dose, to make a real difference. Our clinicians take the time to consider you as an individual to prescribe exercise that reflects current best practice, so you can get the most out of life. What Is Involved?

Building Nutritional Reserve - Dietetics

Our Dietitians work closely with you to identify any small but meaningful changes to your diet that may optimise your function, including building strength and resilience. Unfortunately a very common sign of aging is the process of sarcopenia, in which over time people can slowly lose muscle mass. This can occur often after the age of 30 years but accelerate after the age of 50 years. The slow loss of muscle mass is often difficult to discern as body weight may remain the same, but activities often feel harder to achieve. The timing and volume of Protein within our diet can play a significant role in minimising or reversing muscle mass loss. Combining exercise with an optimise Diet is a great way to improve our health and assist in becoming more robust. Indeed the most feedback our program received from participants is that they did not realise how important the amount and timing of protein was as they get older. What Is Involved?

Building Cognitive Reserve - Occupational Therapy

A New notion to most participants is that like physical health, we may be able to improve of cognition or brain health with activity and exercise. Our clinicians assist you to find ways to challenge the way you think to assist in living a brain healthy lifestyle. Sometimes it may be taking up an old hobby or beginning a new one, our clinicians will get to understand the activities you like and resonate with to find ways to assist in building cognitive reserves, and live a brain healthy lifestyle. Supporting Mental Health and Wellness

Our mental health clinicians can assist you to develop mental health wellness strategies to maintain mental health and wellness. Additionally they can assist you in the early phase of navigating the mental health space. To learn more click here.

What Is Involved?

Group Activities

We offer 9 group sessions in which our group facilitator presents contemporary health promotion information along with encouraging peer to peer information exchange. We understand there is an immense amount of lived experience within our participant cohorts, and we encourage you to share your knowledge with other participants. Our group sessions provide a framework to build new and re- establish old friendships, whilst exercising and learning more about contemporary health and wellness ideas oriented towards seniors. Program Objectives

To provide participants within the program and their families with the tools and information they need to make lasting changes to their health and wellbeing.

Primarily this program aims to:

  1. Reduce the need for institutional care. Keep people in their own home, and active in the community.
  2. Reduce the frequency of hospital admissions and reduce the duration of any admission.
  3. Reduce the frequency of avoidable falls, and severity of falls related injuries.
  4. Promote a healthy mind and body lifestyle to manage mental health.


Recent clinical research has been conducted that suggests that we can halt and even reverse the changes associated with frailty¹. Programs that have the greatest success in improving vitality and reversing signs and symptoms associated with frailty have a number of team members that bring their own expertise and work together to achieve great results. These team members combine their knowledge of exercise, nutrition and cognitive training². The team members of The Vitality Passport include:

  • Physiotherapist/Exercise Physiologist

  • Mental Health Clinician
